What are the factors that affect hair transplant results?
People want to have hair transplants to feel younger and happier. However, does everyone achieve the desired happy outcome after their hair transplant? Unfortunately, the answer to this question is no. So, what are the factors that affect hair transplant results? Why don’t all individuals get the desired results? And what needs to be done to achieve a successful outcome? In this article, we will try to answer these questions. Enjoy reading!

A hair transplant is successful if all three of these things are true.
- Harvesting grafts without disturbing the natural appearance of the donor area: The donor area capacity is different for everyone. If this capacity is exceeded and an excessive number of grafts are taken, distortion in the donor area is inevitable. As a result, areas of thinning or bald spots may form in the nape area, affecting the overall appearance.
- Achieving the targeted and satisfactory hair density in the recipient area: For this, the majority of the transplanted grafts must successfully take root, and the follicles need to regenerate by the end of the process. If the graft survival rate is low, a sparse and insufficient density will result.
- Creating a natural and undetectable appearance: Once a hair transplant is completed, anyone who doesn’t know you should not be able to tell you’ve had the procedure. If they can tell, you won’t be happy with the outcome of the transplant.
In a hair transplant where these three criteria are not fulfilled simultaneously, it would not be correct to speak of success. The main objective of the procedure is to increase hair density in the recipient area in a natural-looking way.
In order to achieve good results from a hair transplant, mistakes must be avoided in the three stages outlined below. In fact, these stages are interconnected, and an error made at an earlier stage will have a chain effect on the subsequent ones. These stages are:

In our opinion, the most important stage in achieving the desired result in a hair transplant is the very beginning of the process—that is, deciding whether you are a suitable candidate for the operation. Whether you are suitable for a hair transplant can only be determined after an examination by an experienced hair surgeon. During the consultation, the points listed below should be taken into consideration.
- Your expectations from the procedure
- Your age
- The cause and extent of hair loss in your family and yourself, and whether your hair loss is still progressing
- Your donor area capacity
- Any chronic conditions and medications you are taking, which your doctor will review in detail

The correct or incorrect actions taken during the operation phase directly affect the outcome of the surgery. In fact, the experience and knowledge of the surgeon performing the procedure and the assisting staff are paramount in this case.
1 ) Choosing the Right Doctor and Clinic
Focus on who will perform the surgery rather than where it will be done. A chain of mistakes made during hair transplantation can lead to undesirable and irreversible outcomes. An experienced hair surgeon with an aesthetic perspective will carry out every stage of the hair transplant according to medical standards, from start to finish. We strongly recommend that you thoroughly review the experience of the doctor performing your hair transplant, the before-and-after results, and the reviews posted on online forums. To achieve a successful hair transplant result, we advise you to consider the differences between experienced hair doctors and black-market or underground clinics.

2 ) Graft Extraction from the Donor Area
Before a hair transplant, the grafts to be implanted are harvested from the donor area. However, graft harvesting also has its own rules. If an excessive number of hair follicles are taken unconsciously beyond the donor area’s capacity, it becomes a case of “trying to fix one thing and ruining another.” Therefore, a balanced number of grafts should be harvested according to each individual’s graft capacity.
To obtain a higher number of grafts, one should not go beyond the safe donor zone. This is because, as harvesting moves outside the safe zone, the anagen (growth) phase of the grafts becomes shorter. In contrast, hair collected from within the safe zone has a survival rate of over 80% after 10 years. Even if hairs with a short anagen phase are transplanted, their survival rate will be lower. Moreover, in the areas where these weak and lifeless hairs are transplanted, the risk of future hair loss increases. As seen, harvesting grafts from the donor area in accordance with proper rules directly affects the outcome of a hair transplant.
The number of grafts required varies depending on the degree of hair loss. For example, in Norwood 3 hair loss, very good results can be achieved with around 2,000 grafts. However, in individuals with extensive baldness such as Norwood 6–7, the required number of grafts may increase up to 10,000.
A hair transplant procedure progresses within a framework of mathematical principles. The number of grafts that can be harvested differs according to each person’s donor capacity. There may even be individuals from whom 10,000 or more grafts can be obtained based on their donor capacity. Of course, harvesting 10,000 grafts in a single session is a major mistake. The most appropriate approach is to divide the harvesting into several sessions. Therefore, in individuals who require a high number of grafts, the hair density achieved in the recipient area will also be mathematically higher.
3 ) Creation of Hairline
Everyone’s hairline is unique to their age and facial structure. An experienced hair transplant doctor knows how to create the most natural hairline suited to the individual. Additionally, the front hairline should consist entirely of single and fine hair follicles. Unfortunately, you may see many people around you with unnatural-looking hairlines due to being filled with hair follicles at steep angles.
4) The Direction and Angle of The Transplanted Hair

The natural exit angles of the hair are 20–25 degrees in the frontal hairline and 10–15 degrees in the temple area. Toward the back, these angles gradually increase. If a hair transplant is performed randomly without respecting the natural angles and directions of hair growth, a good result cannot be expected.
There must be a logical proportion between the size of the area to be transplanted and the number of grafts. In other words, an ideal number of grafts should be implanted per 1 square centimeter. Sparse implantation does not lead to a good result. Attempting excessively dense implantation in a single session is also risky. It increases the risk of necrosis, and because damage to the capillaries that nourish the hair follicles may occur, a large proportion of the transplanted hairs may not grow even if they are implanted densely. Implanting 40–50 grafts per square centimeter provides an ideal density. If a denser appearance is desired and the donor capacity is sufficient, it is more appropriate to perform a second session one year later.
5) Number of Grafts Implanted and Operation Duration
In hair transplantation, the harvested grafts must be implanted into their new locations as quickly as possible, because the time that the follicles can remain outside the body is limited. Therefore, as the number of grafts increases, the duration of the hair transplant procedure also becomes longer. For this reason, a single hair transplant session should ideally be completed within a maximum of 6–7 hours. According to the medical literature, operations lasting longer than 8 hours are associated with a higher rate of graft loss. Consequently, as the procedure time increases, the survival rate of the transplanted hair decreases, leading to a sparser final result.
Generally, between 2,000 and 2,500 grafts can be transplanted within 6–7 hours. For this reason, at AHD Clinic, when the planned graft number exceeds this range, it has become routine practice to divide the hair transplant into two consecutive days. For example, in a patient planned for a 4,000-graft hair transplant, 2,000 grafts are harvested and implanted on the first day, and the same procedure is repeated on the second day with another 2,000 grafts. From a commercial perspective, this is actually a disadvantage for us, because we could treat another patient the following day. However, we believe this approach is medically correct and we do not compromise on quality.
The reasons listed so far are related to the doctor and the technicians and have a direct impact on the final outcome of the hair transplant.
6) Technique Used
As you know, FUE and DHI techniques are commonly and popularly used in hair transplantation. In fact, if done according to the proper rules, it is possible to achieve successful results with both techniques. At AHD Clinic, we have been using the DHI method for all of our applications such as hair, beard, and eyebrow transplants since 2010. This is because we have found that the satisfaction rate is higher in patients who undergo hair transplantation using the DHI method due to its advantages. When it comes to implanter pens and DHI hair transplantation, AHD Clinic is the first place that comes to mind in Turkey.
However, we would like to emphasize this point. The hair transplant pens used in this technique are not a magic wand. What is important is the technical knowledge and skill to use the tool in the best possible way, which directly impacts the result of the hair transplantation.
7 ) The Nature and Quality of Grafts
We must keep in mind that even if the same number of grafts are transplanted, a hair transplant will not yield the same results for everyone. Each hair follicle extracted from the donor area may contain a different number of hair strands. Some grafts may have 1, some 2, some 3, and some may even have 4 or more hair strands. Therefore, consider three individuals who receive 3,000 grafts. Even though the number of grafts is the same, the number of hair strands could be 4,000 for the first person, 6,000 for the second, and 8,000 for the third. Consequently, the hair density achieved in the third person will be twice that of the first person.
8) Structure and Color of Hair
The structure of a hair follicle can vary from person to person and may be thick, thin, or of normal volume. Hair can also be straight, wavy, or curly. Thick and wavy hair naturally creates a fuller and denser appearance.
Likewise, hair color can differ among individuals; it may be black, blonde, red, brown, or white. Variations in hair color and tone can also lead to differences in results, which is completely normal.
The Effect of the Post-hair transplant Period on Results
After a hair transplant, expected complications such as redness, scabbing, pain, swelling, and shock loss will occur. Most of these are temporary conditions that will resolve with simple precautions. Especially during the first 10 days of the recovery period, mistakes can negatively affect the outcome of the hair transplant.
In particular, during the first month, you should take the right steps regarding exercise, sexual activity, sleep, sleeping position, swimming, sun protection, and haircuts. Therefore, you must follow your doctor’s recommendations during this period.
Additionally, if sterilization rules are not followed in the donor and recipient areas, an infection may develop. If the infection cannot be controlled, the transplanted follicles may die. Due to the lost follicles, the desired hair density may not be achieved.
Hair Transplant Results: A Monthly Timeline
It is necessary to wait for a certain period for the transplanted hair to reach the desired level. What kind of changes will you experience month by month after the hair transplant? Let’s take a look at this now with the photos.
Month 1

Especially the first 10 days are the period of wound healing. During this time, it is normal to experience mild and temporary complications such as pain, redness, scabbing, and swelling. With the first wash, the scabs gradually begin to fall off. From the second week onward, the transplanted hair starts to shed. This condition, known as shock loss, is normal and temporary. Mistakes made especially within the first 10 days can negatively affect the hair transplant result. Therefore, it is essential to follow your doctor’s post-operative instructions. During the first two weeks, you should be careful with activities such as sports, trauma, hair cutting, washing, sexual activity, as well as sleeping and lying positions.
2. Month

Shock loss may continue for up to about two months. In fact, you have entered the period when you may feel the most unhappy. You might become anxious because of the hair you are losing. Additionally, due to a non-uniform appearance, visual irregularities can occur during this phase. However, please do not worry—this is only a temporary condition.
3. Month

Shock loss has been completed. During this period, irregular and non-uniform appearances are observed. The hairs that have penetrated the skin begin to enter the emergence phase. From the 3rd month onward, the shed hairs start to regrow gradually, one by one.
Months 4–5

By the end of the 4th month, around 30% of the transplanted hair has typically started to grow. However, the first hairs that appear are usually thin and sparse. As blood circulation increases, the hair follicles will be better nourished. During this period, scalp massage can be beneficial as it helps improve blood flow.
In the 5th month, hair growth continues to increase, but the desired final result has not yet been achieved.

Months 6–8
By the end of 6 months, approximately 60% hair growth can be expected. However, the desired hair transplant result has not yet been fully achieved. Hair growth continues to increase, and regrowth in the recipient area begins to accelerate. Around the 7th–8th months, the initially fine hair shafts also start to thicken.
9 months

By the end of 9 months, approximately 80–90% of hair growth is completed.
12th month (1 year)

The required period to achieve the desired hair transplant result is one year. By the end of 12 months, approximately 95% of hair growth is completed. At this stage, the hair will have almost reached its natural appearance. For transplants performed in the crown area, a period of up to 18 months is required to achieve 100% results.
Evaluating of The Results
The result of a hair transplant operation is unique to each individual. When comparing two people who have had a hair transplant, it is important to consider the evaluation criteria as well. This is because it is not correct to expect the same hair transplant result for everyone. For example;
- A person with Norwood Type 3 hair loss can achieve a very successful result with 2500-3000 grafts. However, in someone with a wider hair loss pattern, like Norwood 6, even with 5000 grafts, a good density might not be achieved.
- Even if the same number of grafts are transplanted, different results can occur between two individuals due to factors like hair color, hair thickness, and whether the hair is wavy or straight. In other words, when making comparisons, it should be apples to apples, not apples to oranges.
- Medications such as Fi*nast*eri*de and Mi*nox*idil, which are used as post-operative support, can contribute positively to the results.
